Please upload a full body profile picture of your dog(s). The dog(s) should be fully upright with profile of the body to the right or left. Additionally, please upload a clear photo of your dog(s) face. Please label each photo with the dog(s)’s name.

This form can be downloaded from the CDC website. Please upload your dog(s) Certification of Foreign Rabies Vaccination and Microchip (CFRVM) here. This specific form is required to enter the United States. Other certificates and forms, including pet passports and origin country rabies certificates, are not accepted. The certification(s) must be in English. Each dog listed for importation must have an individual CFRVM uploaded here.

Please upload your dog's Rabies vaccine titer lab work results here, if available. Each dog listed for importation must have an individual titer results uploaded here. If no results are uploaded, we will assume you do not have Rabies vaccine titer for your dog(s).

All boarders at Gateway Animal Care Center must be up-to-date on Rabies, Distemper, and Bordetella vaccines, have negative fecal parasite testing, as well as endo- and ecto-parasite prevention. Please provide all of your pet(s) vaccination information and recent veterinary records HERE for our review. We will provide you with an estimate for any additional veterinary care required upon arrival for boarding at our facility.
